چکیده

background: in this study we evaluated the incidence of hypothyroidism in patients who received external beam radiation due to cancer, when radiation portals included thyroid gland. methods: this prospective study was conducted from april 2004 to december 2005 among patients with head and neck or breast malignancies who referred to radiation oncology department of cancer institute, imam khomeini hospital, treated with external beam radiotherapy. all patients received radiation to the thyroid gland. thyroid function was tested at the beginning of treatment, 3 months and 6 months after the completion of radiotherapy. results: in all 34 patients were included in the study. the median age of patients was 53 years. eighteen patients were female and 16 were male. all patients received external radiotherapy. they received radiation ranging from 5000 cgy (14 patients 41.2%) to 7400 cgy (1 patient 2.9%) with median of 6000 cgy. physical examination of thyroid at the beginning of treatment was normal and all of the patients were euthyroid . the results from tsh test showed significant difference at baseline, after 3 and 6 months follow-up (p = 0.001) conclusion: the findings indicated that none of the patients developed hypothyroidism in a period of six months after completion of radiotherapy. however, it seems that 6 months fallow-up is early for judgment about evaluation of patients for hypothyroidism state and it needs further follow up for minimum of 12 months.

Thyroid cancer after exposure to external radiation: a pooled analysis of seven studies.

The thyroid gland of children is especially vulnerable to the carcinogenic action of ionizing radiation. To provide insights into various modifying influences on risk, seven major studies with organ doses to individual subjects were evaluated. Five cohort studies (atomic bomb survivors, children treated for tinea capitis, two studies of children irradiated for enlarged tonsils, and infants irra...

Correlative study of dose to thyroid and incidence of subsequent dysfunction after head and neck radiation.

BACKGROUND Thyroid dysfunction is common after radiotherapy (RT) for patients with head and neck cancers. We attempted to discover RT dose parameters that correspond with RT-induced thyroid dysfunction. METHODS Records of 102 patients who received RT from 2008 to 2010 were reviewed with respect to thyroid function. Abnormalities were grouped in 2 ways: (1) none, transient, or permanent; and (...

Hypothyroidism after radiotherapy for primary thyroid lymphoma.

BACKGROUND Thyroid lymphoma is rare, with no standard therapeutic protocol. Radiotherapy is often employed. PATIENTS AND METHODS Nine patients who had received radiotherapy for thyroid lymphoma in our Institute, from September 2005 to August 2013, were reviewed for hypothyroidism. The median radiation dose was 40 Gy. Thyroid-stimulating hormone (TSH) levels were measured before and after radi...

External Beam Radiation in Differentiated Thyroid Carcinoma

The treatment of differentiated thyroid carcinoma (DTC) is surgery followed in some cases by adjuvant treatment, mostly with radioactive iodine (RAI). External beam radiotherapy (EBRT) is less common and not a well-established treatment modality in DTC. The risk of recurrence depends on three major prognostic factors: extra-thyroid extension, patient's age, and tumor with reduced iodine uptake....

Radiation-induced Hypothyroidism in Head and Neck Malignancy

Background and Objective: Hypothyroidism is a known consequence of the external beam radiotherapy to the neck encompassing the thyroid gland for over 40 years. The aim of this study was to find out the incidence of hypothyroidism in patients with head and neck cancer treated with radiotherapy, when radia...

Discriminating Gene Expression Signature of Radiation-Induced Thyroid Tumors after Either External Exposure or Internal Contamination

Both external radiation exposure and internal radionuclide contamination are well known risk factors in the development of thyroid epithelial tumors. The identification of specific molecular markers deregulated in radiation-induced thyroid tumors is important for the etiological diagnosis since neither histological features nor genetic alterations can discriminate between sporadic and radiation...
